Referral Forms

For Veterinarians

Patients are accepted on a referral basis for advanced dentistry and oral surgery procedures. These include, but are not limited to, root canal therapy, orthodontics, jaw fracture repair, treatment for stomatitis, advanced periodontal disease therapy, and major oral surgery such as cleft palate repair and oral tumor removal.

If you would like to refer a patient, you can click here to download the referral form. Please complete the form and fax it to 800-646-9352.

We highly recommend pre-operative blood work. If this can be done at your office, it is preferable. However, it can be performed by Dr. Bannon if necessary.

 - If your patient is less than 7 years of age, a CBC and 6-chem panel within the last 6 months would be acceptable.

 - If your patient is 7 years or older, we prefer a CBC, complete chemistry profile and a thyroid screen within the last 30 days.

 - If your patient has oral cancer or a suspicion of cancer, we recommend 3-view thoracic radiographs prior to the tumor removal.

  - If your patient has any special anesthetic concerns (heart murmur, seizures, kidney disease, etc) please let us know in advance of the appointment on the referral form. There are very few conditions that will completely prevent anesthesia for dental therapy, but modifications are likely and arrangements may need to be made prior to the appointment.

For Clients

If you have been referred to Dr. Bannon by your regular veterinarian, your pet has a dental or oral condition that your veterinarian feels requires specialized care. You may complete our new patient information sheet prior to your arrival in our office.

Click here to download this form. You may complete it at home and bring it with you, or you may fax it to 800-646-9352. You may also email the form to Melissa at toothtech@vetdentistrynm.com

Your appointment may be for a consultation only, or a consultation followed by a procedure the same day. If you are not sure which type of appointment you have, please call 505-471-0747 to find out. If you do not yet have an appointment, please call to make arrangements. We prefer to schedule everything in one visit if that is convenient for you. However, if you are unsure that you are likely to commit to a procedure prior to speaking with Dr. Bannon, you may wish to schedule a consultation only.

 - If you have an appointment for a consultation and examination only, there are no special considerations for feeding or allowing water the morning of your appointment. Please bring your pet, and any x-rays or copies of laboratory tests (blood tests, biopsy) that your veterinarian has given you.

  - If you have an appointment for a consultation followed by a procedure, do not feed your pet breakfast the morning of your appointment. Water is acceptable all night, but no food after 10pm the night prior to your appointment. Please bring any x-rays or copies of laboratory test (blood tests, biopsy) that your veterinarian has given you. Also bring with you any medications that your pet is currently receiving. This allows Dr. Bannon not only to know what your pet is taking, but we can give the medication if necessary during your visit.

If you have any questions, please do not hesitate to call 505-471-0747.
